Report a Concern

Tell us something has gone wrong. Anyone can use this form, and you do not have to give your name.

If someone is in danger right now, do not use this form. Call 911. To report suspected abuse, neglect, or financial exploitation of an adult you can also call Illinois Adult Protective Services on 1-866-800-1409, any hour of the day. You do not have to tell us first, and you do not need our permission.
How this works

We would rather hear it

A complaint tells us something we would not otherwise know. It is logged the day it arrives, before anyone decides whether it has merit.

  • No one loses service over a complaint. Nobody's care will be reduced, delayed, or stopped because a report was made, and no employee will be disciplined for making one.
  • You will hear back. We acknowledge in writing within two business days and give you an outcome within 30 days.
  • You can stay anonymous. We will still investigate it — we just will not be able to send you the outcome.
You can also go elsewhere first. Raise it with the caseworker who arranged the service, or with the Illinois Department of Public Health on 1-800-252-4343. You do not have to come to us at all.
